Cardiology — CK
A 68-year-old man with HFrEF (EF 25%) on optimal GDMT now has worsening dyspnea. K is 5.6. What's the next step — and why is this counterintuitive?
Add an SGLT2 inhibitor. Despite the elevated K, SGLT2 inhibitors have a slight K-lowering effect via diuresis and have proven mortality benefit in HFrEF (DAPA-HF, EMPEROR-Reduced). Don't reflexively pull back the ARB/spironolactone — the SGLT2i is independently indicated.
Pulmonology — CK
A patient with a recent total knee replacement has unilateral leg swelling and a positive D-dimer. Lower extremity ultrasound is negative. What's the next test — and what are you ruling out?
Get a CT pulmonary angiogram. Negative LE U/S doesn't rule out a calf-vein DVT or pulmonary embolism, especially in this very high pre-test probability setting. The post-op PE risk persists for weeks. Don't anchor on the negative U/S.
