Cardiology — CK

A 64-year-old man with HFrEF (EF 30%) takes lisinopril, carvedilol and spironolactone. Routine labs show K 6.1. His ECG is unchanged. What's the next step?

Stop the spironolactone and recheck potassium. MRAs are the most common cause of hyperkalemia on guideline-directed therapy: reduce or hold the dose above 5.5 and stop it above 6.0, then restart cautiously once K is back in range. In HFrEF, the potassium decides the MRA dose.

Pulmonology / OB — CK

A 31-year-old woman, 5 days postpartum, has sudden dyspnea and pleuritic chest pain. HR 118, SpO2 91%, BP 122/74. What's the next step — and which test should you skip?

Get a CT pulmonary angiogram. Pretest probability is high, so skip the D-dimer: it's physiologically elevated in pregnancy and postpartum, so a positive result tells you nothing. She's hemodynamically stable, so this is imaging, not thrombolysis. Start heparin while you wait if suspicion is high.
